SUPERVISION

We are pleased to support clinicians working toward LPC licensure and/or RPT certification. Please review the information below to learn more about the application process!

  • If you are a clinician interested in becoming a Registered Play Therapist, please contact Ashley Wade by sending your resume and/or curriculum vitae to awade@yellowcityplayroom.com and requesting an interview.

  • Successful completion of the credentialing requirements demonstrates the following:

    • Instructional, clinical experience, and supervision hours must be accrued in a time period of no less than two (2) years and no more than seven (7) years.

    • Each phase requires the completion and documentation of a minimum number of hours in play therapy instruction, clinical experience, and supervision prior to progressing to the next phase.

    • All three phases must be completed before submitting application.

    This process is designed to aid in obtaining comprehensive knowledge of the theories and clinical skills required for the competent practice of play therapy.

  • Ashley Wade provides mentoring, training, and supervision to new LPC Associates entering the field of professional counseling.

  • Below is a checklist of documentation that is required for LPC licensure. For more detailed information please click HERE.

    All Applicants:

    • Completed Online Application

    • Application Fee. See 22 TAC 885.1 for a list of the fee amounts.

    • Practicum Documentation (must be on Texas form) *

    • Proof of Completion of Texas Jurisprudence Exam for Professional Counselors*

    • Official NBCC score report for NCE or NCMHCE (NBCC sends these to us electronically) **

    • Official Transcript showing conferral of degree—must be sent to board by email from university/sealed envelope from university**

    • Self-query report from NPDB (either a certified self-query report sent by email, or a printed one received in sealed envelope from NPDB)

    • SAVE form, if applicant is not a US citizen

    Additional Item Required for LPC-Associate Only:

    • Supervisory Agreement Form*

    Additional Items Needed if Applicant Is Licensed in Another Jurisdiction:

    • Official Verification of Licensure in other Jurisdiction sent to board by other state**

    • Proof of completed supervised clinical experience submitted**

    The confirmation email you receive after submitting your online application includes a link to information on how to submit your fingerprints for the background check.

    * These items should be uploaded as attachments to your online application.
